Sick Bay Procedure

Schedule of Activity Fees 2022

If a student needs to be permanently administered medication, a parent needs to complete this form and return to school.

If a student needs to be temporarily administered medication, parents need to complete this form and return to school.

For 5+ days parents must complete and submit an application for approved leave. Application must be received prior to the student commencing leave. If approved a certificate for leave will be sent home.

Change of Contact Details

If a student wants to ride their bike or scooter to school, parents need to complete this form and return to school.
